W opracowaniu przedstawiono aktualny stan wiedzy w zakresie stosowania leków przerywających przewodnictwo nerwowo-mięśniowe u dzieci leczonych respiratorem. Omówiono leki dostępne w praktyce klinicznej pod kątem ich użycia w intensywnej terapii, ich zalety i powikłania ich stosowania. Z uwagi na dużą ilość notowanych powikłań zalecono stosowanie ich tylko w wyjątkowych przypadkach (LOE 3, rekomendacja D), głównie do intubacji (2C). Zwrócono uwagę na konieczność zapewnienia odpowiedniej sedacji zwiotczanym dzieciom (LOE 3 i 4, rekomendacja D). Lekami polecanymi do krótkotrwałej blokady nerwowo-mięśniowej u niemowląt są atrakurium i cis-atrakurium, te ostatnie z uwagi na nieuwalnianie histaminy i samoistny rozpad w ustroju. (LOE 2++, rekomendacja B).

Objectives. Neuromuscular blocking agents (NBA) are sometimes used to facilitate mechanical ventilation; elimination of respiratory effort has potential advantages in this aspect. In this paper, advantages and complications of prolonged neuromuscular relaxation are reviewed and discussed. Available neuromuscular blocking agents are presented.

Recommendations. Because of considerable high number of complications associated with prolonged relaxation and lack of evidence from randomized trials the routine use of NBA’s in infants and children receiving mechanical ventilation cannot be recommended and should be limited to carefully selected cases only (LOE 3, recommendation grade D), mainly for endotracheal intubation (recommendation grade 2C). Adequate sedation should be provided to the paralyzed children (LOE 3 and 4, recommendation grade D). For short-term muscle relaxation, atracurium and cis-atracurium are recommended, due to their organ-independent elimination, and in the case of cis-atracurium, lack of histamine release. (LOE 2++, recommendation grade B).
